What could be causing my toe nail (big one) to be growing off with new nail underneath?

No pain, infection or injury...new nail coming in seems fine...

It looks like you are infected with nail fungus. I had pretty similar situation as you have. So I went to see my doctor and he told me it was nail fungus. I took laser treatment recently, so far the effect is good but still need time to examine the final result. But I still suggest you to ask the professionals. Welcome to EmpowHER. Many things might be the cause of this, a trauma to the nail, nail fungus, brittle nails and many other.  You need to see a doctor and have him examined it, so you can get the proper treatment for this.
