Miscortes, apparently the mastectomy bras that stay in place the best have extra banding around the bottom of the bra. One website noted that Playtex now makes a mastectomy bra that has custom fit and self-adjusting features. There are stretch straps for smaller bust sizes and non-stretch straps for larger breast sizes. It has a pocket for a prosthesis and an underbanding to hold the bra in place.

There is a certification for fitting bras after a mastectomy, called Certified Mastectomy Fitters. They've had special training by the companies that make post-mastectomy undergarments. In addition to helping a woman get the perfect fit, they understand support and symmetry issues in the body.
CMF work in many places -- large department stores and lingerie stores, for instance. A woman can just call ahead and ask if they have someone on the staff who is certified in fitting bras after mastectomy, and make an appointment with that person.
