I don't think an inexpensive, nice bottle of wine from the patient's region or a seasonal basket of fruit is a problem at all. Anything more expensive (or cash) would be inappropriate.
My problem is with pharmaceutical reps giving 'gifts' and I would be concerned about (and be distrustful of) a doctor who would accept them. I know it goes on all the time and it doesn't sit well with me.
